The following are official FAA reports of the midair collision between Frank Beran and Charles Peterman. Since the airport is not a controlled facility, the FAA did not levy any punishment.
After reading the reports, I would place the majority of the blame on Peterman. He was flying a non-standart approach. Apparently, this was a fairly common practive of Peterman. As many in the aviation community have commented, this practice leads to an accident waiting to happen. Additionally, I, personally know that Frank Beran announced his intentions each time he entered the pattern at any airport.
The last figure is for Peterman's fatal accident. Draw you own conclusions as to how well the aircraft was maintained.
